Thanks for inviting one of CPMC's Conference Ministers to be with your congregation on a Sunday morning. In preparation for the visit, please fill out this form and send the following to [email protected] at least one week in advance of visit.

    1) a photo of the church building
    2) A (relatively) recent photo of your congregation doing something together (worship, work, fellowship, etc.)
    3) If a CPMC representative is coming to license/install/ordain a pastor or leader, please provide a picture of that leader

    *Your submission of these photos will serve as permission for CPMC to use them on social media, in publications, etc.
